How this instrument works

Frame size is a rough proxy for how much of a person's weight is skeleton rather than fat or muscle, estimated from a spot where a tape measure sits almost directly on bone: the wrist, just past the styloid processes. Divide height by that circumference and the units cancel, leaving a dimensionless number — commonly called the Grant index in clinical nutrition literature, after J.P. Grant's 1980 nutritional assessment handbook, which popularised the ratio for bedside use where a skinfold caliper or bone-density scan was not available.

The ratio itself is not a verdict — it is a lookup key. Reference tables built from population data assign bands to the number: on the commonly cited chart, men above roughly 10.4 are classed small-framed, 9.6 to 10.4 medium, and below 9.6 large; women's bands sit about half a point higher because women's wrists average narrower relative to height. This instrument computes the ratio precisely; matching it to a band is a separate step, because the cutoffs are only ever approximate and more than one table circulates — MedlinePlus, for instance, publishes a chart that checks wrist size directly against height brackets rather than dividing the two.

None of this measures bone density, flags a skeletal condition, or replaces a scan ordered by a clinician. Wrist circumference includes a thin layer of skin and tendon along with bone, swelling from injury or fluid retention can inflate it temporarily, and the reference bands were built from adult population averages that take no account of age, ethnicity or unusual proportions. Frame size exists mainly as a correction factor for older ideal-weight tables, which assumed a medium frame and needed adjusting for people who were not — it was never meant to stand alone as a health measurement.

r is height divided by wrist circumference, both measured in the same unit; the instrument converts your chosen units to a common scale before dividing, so the ratio itself carries no unit.

Worked example — 170 cm height, 17 cm wrist

Set Height to 170 centimetres and Wrist circumference to 17 centimetres — a wrist close to the width of a typical adult watch strap. The instrument divides 170 by 17 and returns a Height-to-wrist ratio of exactly 10.0000, chosen as the reference case because the arithmetic checks by hand in seconds: 170 divided by 17 is 10.

That figure lands almost exactly in the middle of the medium-frame band most charts use for men, which runs from 9.6 to 10.4 — 10.0 is its midpoint. On the women's table, whose bands sit roughly half a point higher, the same 10.0 falls just past the 10.1 cutoff into the large-frame range. Identical measurements read against two different reference tables land in two different bands, which is the clearest reason the raw ratio needs the right table beside it rather than one universal rule.

How accurate is the height-to-wrist ratio for frame size?

It is a rough screening ratio, not a measurement of bone mass. Wrist circumference includes skin and tendon along with bone, swelling can inflate it temporarily, and the small, medium and large cutoffs come from population averages rather than an individual scan. Treat the band it lands in as a loose descriptor, not a verdict on skeletal size.

Why do men and women get different cutoffs for the same ratio?

Because women's wrists average narrower relative to height than men's, a ratio that reads medium for a man can read large for a woman. Reference tables split the bands by sex, roughly half a ratio point apart, so a 10.0 sits centrally in the men's medium band but past the boundary into the women's large band.

Why doesn't the calculator just label me small, medium or large?

Because the cutoffs are not universal — several reference tables circulate with slightly different boundaries, and some check wrist size against height brackets instead of a ratio at all. The instrument reports the precise ratio; comparing it to whichever table a clinician or trainer trusts is a deliberate extra step, not an omission.

Can weight gain or loss change my frame size?

Genuine skeletal frame does not change once growth plates close, but the measured ratio can still drift: significant weight change alters wrist circumference slightly through fat and fluid around the joint, and inconsistent tape tension between readings shifts the number more than most people expect. Measure snugly, not tight, for a repeatable figure.

What is frame size actually used for?

Mainly as a correction for older ideal-body-weight tables, such as the Metropolitan Life tables, which quoted a weight range for a medium frame and expected users to adjust up or down for a small or large one. Modern practice leans on BMI and waist measurements instead, so frame size today is mostly a historical footnote revived by fitness calculators.

Why does my doctor never mention my frame size?

Because it adds little to modern clinical assessment — BMI, waist circumference and, where warranted, a bone-density scan give more direct information about body composition than a wrist-to-height ratio ever can. Frame size persists mostly in fitness and strength-training contexts, not on medical charts.
