How this instrument works
This score scores six values most hospitals already draw for a patient with a concerning soft-tissue infection: C-reactive protein, white blood cell count, hemoglobin, sodium, creatinine, and glucose. Each contributes points on its own band — CRP alone can add 4 of the 13 possible points, since a markedly elevated reading was, in the original data, the single strongest signal — while the other five contribute smaller amounts as they drift further from normal. Add all six and the sum runs from 0 to 13.
Wong and colleagues built the tool in 2004 from a retrospective analysis of confirmed necrotizing fasciitis cases against ordinary cellulitis and abscess admissions, publishing it in Critical Care Medicine as a way to flag the rarer, faster-moving diagnosis using labs clinicians were often drawing anyway. Their data suggested three rough bands: a total of 5 or below carries a low probability, under roughly half; 6 or 7 sits in an intermediate range, historically cited around 50-75%; and 8 or above is high risk, over roughly three-quarters. The 6-point mark gets cited often on its own too, as a practical cue to raise suspicion even before a patient reaches the 8-point high-risk tier.
None of those figures should ever slow down a surgeon's hand. Necrotizing fasciitis kills through delay, and the disease can progress from subtle to catastrophic within hours — clinical judgment and prompt surgical exploration belong ahead of any lab-based reading when the picture looks concerning, not behind it. Wong and colleagues' own validation found the tool missed roughly one in ten confirmed cases, a real false-negative rate worth remembering: a reassuringly low total does not clear a patient who otherwise looks sick, and should never be read as permission to wait and watch.

Worked example — a total of 13, the maximum score
A CRP of 200 mg/L clears the ≥150 threshold for the full 4 points. A WBC of 30 clears 25 for 2 points. A hemoglobin of 9 g/dL falls under 11 for 2 points. A sodium of 130 mmol/L falls under 135 for 2 points. A creatinine of 2.0 mg/dL clears 1.6 for 2 points. A glucose of 200 mg/dL clears 180 for the last point. Every band maxes out: 4+2+2+2+2+1 = 13, the highest reading the scale allows and a strong flag for necrotizing fasciitis.
A more borderline case shows how much smaller changes matter. A CRP of 160 mg/L (4 points), WBC of 18 (1 point), and hemoglobin of 12 g/dL (1 point), with sodium, creatinine, and glucose all normal, sums to 4+1+1 = 6 — the commonly cited threshold for raising clinical suspicion, well short of the 8-point high-risk tier but already past the point of reassurance.
Questions
What LRINEC total should prompt concern?
6 or above is the figure most often cited as a practical cue to raise suspicion for necrotizing fasciitis, though the original study's own risk tiers put 6-7 at intermediate probability, roughly 50-75%, and reserve high risk, over roughly 75%, for 8 and above. Either way, a rising or elevated reading in a patient who looks clinically unwell warrants urgent surgical evaluation, not a wait-and-recheck approach.
Can a low LRINEC score rule out necrotizing fasciitis?
No, and this is the tool's most important limitation. Wong and colleagues' own validation study found it missed roughly 10% of confirmed necrotizing fasciitis cases — a real false-negative rate. A patient who looks clinically concerning, with pain out of proportion to exam, rapidly spreading skin changes, or systemic illness, needs surgical evaluation regardless of what the number reads.
Should treatment ever wait for a LRINEC result to come back?
No. Necrotizing fasciitis is a surgical emergency where delay measurably worsens outcomes, and prompt exploration and debridement should never be held up while labs are pending if the clinical picture looks concerning. The score is meant to support judgment on ambiguous cases, not to gate a decision that clinical suspicion alone can already justify making.
Why does CRP carry so much more weight than the other five labs?
Because in Wong and colleagues' original 2004 analysis, a markedly elevated CRP was the single strongest independent signal separating necrotizing fasciitis from ordinary soft-tissue infection, worth 4 of the scale's 13 total points on its own. The other five labs — white cell count, hemoglobin, sodium, creatinine, and glucose — each carry smaller weights, 1 or 2 points, reflecting a comparatively weaker independent association.
What conditions can raise the score without necrotizing fasciitis being present?
Any severe infection or systemic illness can elevate CRP, white cell count, or creatinine and nudge the total upward, since none of the six labs is specific to necrotizing soft-tissue infection on its own. That is part of why this tool is framed as a risk indicator to combine with clinical judgment, not a stand-alone diagnostic test.
