How this instrument works
In the earliest days a pregnancy is visible on ultrasound — often around five to six weeks — there's usually nothing to see yet but a small fluid-filled gestational sac; the embryo itself, the fetal pole, hasn't grown large enough to distinguish. Sonographers measure the sac across three perpendicular planes and average them into a single mean sac diameter (MSD), a more stable figure than any one measurement alone, since a sac is rarely a perfect sphere.
A rough rule of thumb — add 30 to the MSD in millimetres to estimate gestational age in days — traces back to Hellman and colleagues' 1969 study of early fetal growth, published in the American Journal of Obstetrics and Gynecology. It's a genuinely old, coarse teaching shortcut, accurate to only about plus or minus 5 to 7 days at best, offered here for context and quick estimation rather than as a precise dating tool. Once a fetal pole becomes visible — usually within another week or so of growth — crown-rump length (this site runs a separate calculator for it) takes over as a meaningfully more accurate way to date the pregnancy, and should be preferred whenever it's available.
MSD carries a second, more clinical role beyond rough dating: a 2013 multispecialty panel led by Doubilet and colleagues, publishing in the New England Journal of Medicine, established diagnostic thresholds for a nonviable early pregnancy, and one of them is a mean sac diameter of 25 mm or more with no embryo visible inside it. That specific cutoff was set deliberately conservative, to avoid ever mistaking a normal, simply slow-to-show pregnancy for a failed one, and real clinical decisions still weigh it alongside a patient's history, hormone trends, and often a follow-up scan rather than a single measurement taken in isolation.

Worked example — three diameters of 15, 16, and 14 mm
Three diameters measured across the sac: 15, 16, and 14 mm. Add them — 15 + 16 + 14 = 45 — and divide by 3 for a mean sac diameter of exactly 15 mm. Add the old rule-of-thumb constant of 30 and the rough estimated gestational age comes to 45 days, or 6 weeks and 3 days — a first approximation only, until more precise dating becomes possible.
A slightly larger sac measuring 20, 22, and 21 mm sums to 63, dividing to an MSD of 21 mm, for a rough estimate of 51 days — about 7 weeks 2 days. A smaller, earlier sac at 10, 11, and 9 mm sums to 30, giving an MSD of 10 mm and a rough estimate of 40 days, early enough that a fetal pole often isn't visible at all yet.
