How this instrument works
A complete blood count with differential reports several kinds of white cells separately, including neutrophils and lymphocytes. Neutrophils surge first during acute bacterial infection or physical stress; lymphocytes often dip under the same conditions, partly through cortisol-driven redistribution out of the bloodstream. Dividing the absolute neutrophil count by the absolute lymphocyte count captures both shifts in one figure — climbing whenever the body is under strain, falling back toward baseline as it settles.
Forget, Khalifa, Defour, and colleagues examined this quotient across two cohorts of healthy working adults and reported a normal range of roughly 0.78 to 3.53, with an average near 1.65, in a 2017 paper in BMC Research Notes. That range gives clinicians something to compare against: a person sitting inside it is unremarkable, while someone well outside it is worth a second look at what else might be going on.
The appeal of this figure is that it costs nothing extra — both counts already come back on a standard panel — and it correlates loosely with severity across a surprisingly wide range of conditions, from pneumonia to heart attacks to certain cancers. The catch is specificity: an elevated result reflects generic physiologic stress, not any single disease, so it works as a supporting clue that prompts further workup rather than as a test that names what is wrong on its own.

Worked example — three complete blood counts
ANC 4.5 ×10³/µL, ALC 2.0 ×10³/µL. Divide directly: 4.5 ÷ 2.0 = 2.25 — comfortably inside the roughly 0.78 to 3.53 range reported for healthy working adults, and not a figure that would raise concern on its own.
ANC 8.0 ×10³/µL, ALC 1.2 ×10³/µL. Neutrophils have surged while lymphocytes have dropped: 8.0 ÷ 1.2 ≈ 6.67, well above the typical range — a pattern seen with acute bacterial infection, significant injury, or major physiologic stress of some other kind.
ANC 3.0 ×10³/µL, ALC 2.5 ×10³/µL. Here 3.0 ÷ 2.5 = 1.2 exactly, near the low end of the usual range and well within what a healthy adult can show without anything being wrong.

Is NLR used to diagnose a specific disease?
No single condition produces this pattern uniquely, so it is not diagnostic by itself. It functions as a cheap, widely available supporting marker — useful for tracking severity or trend over time, or as one input among several in a broader clinical assessment, rather than a stand-alone test that identifies a cause.
What units does this calculator expect for ANC and ALC?
Both counts in ×10³ cells per microliter (equivalently, ×10⁹ per litre), the standard units reported on most complete blood count differentials. Since the result is a simple quotient, the units cancel out entirely as long as both counts are entered using the same scale.
Can a low lymphocyte count alone explain a high ratio?
Yes — the quotient rises whenever either the numerator climbs or the denominator falls, and a depressed lymphocyte count from stress, certain infections, or some medications can push the figure up even with an unremarkable neutrophil count. Reviewing both individual counts, not just their quotient, clarifies which side of the equation actually moved.
