How this instrument works

Serum iron measures how much of the metal is circulating in the blood at that moment. Total iron-binding capacity (TIBC) is an indirect measure of how much transferrin is available to carry it — effectively, the maximum the blood could hold if every binding seat were occupied. Dividing the first figure by the second, then multiplying by 100, gives the percentage of those seats actually filled: transferrin saturation, usually shortened to TSAT.

The figure sits at the center of the American Association for the Study of Liver Diseases' 2011 practice guideline on hemochromatosis, an inherited condition in which the body absorbs and stores too much iron. Bacon, Adams, Kowdley, Powell, and Tavill's guideline recommends a TSAT at or above roughly 45% as a common screening trigger for further workup — additional lab tests, genetic screening, or a liver biopsy in some cases — to check for overload before organ damage accumulates.

The same number reads the opposite direction at its low end. A TSAT well under 20% is one of the standard clues pointing toward iron-deficiency anemia rather than overload, since too little of the metal circulating leaves most transferrin seats empty. Neither reading stands alone, though — TSAT is meant to be interpreted alongside ferritin and the rest of the panel, since inflammation, liver disease, and other factors can shift TIBC and serum levels independently of how much a person actually has stored.

Worked example — three iron panels

Serum iron 100 µg/dL, TIBC 300 µg/dL. Divide first: 100 ÷ 300 = 0.333, then × 100 gives 33.3% — comfortably within the typical normal range, roughly a third of transferrin's binding capacity occupied.

Serum iron 180 µg/dL, TIBC 250 µg/dL. Here 180 ÷ 250 = 0.72, times 100 gives 72% — well above the roughly 45% threshold the AASLD guideline uses as a screening flag, a result that would prompt a closer look at further studies and possibly genetic testing for hemochromatosis.

Serum iron 60 µg/dL, TIBC 400 µg/dL. Dividing gives 60 ÷ 400 = 0.15, or 15% — little of the metal relative to a high carrying capacity, the pattern typically seen in iron-deficiency states rather than overload.

What is transferrin saturation used for?

It estimates what fraction of the blood's iron-carrying capacity is actually occupied, which helps distinguish overload from deficiency. A high reading, alongside other lab studies, screens for hereditary hemochromatosis; a low reading is one of the standard clues supporting a diagnosis of iron-deficiency anemia.

What TSAT level suggests hemochromatosis?

The 2011 AASLD practice guideline on hemochromatosis uses a threshold around 45% as a common trigger for further evaluation, such as additional lab studies or genetic testing. It is a screening cutoff rather than a diagnosis on its own — confirming hereditary hemochromatosis typically also involves genetic testing and, in some cases, imaging or a liver biopsy.

What does a low transferrin saturation mean?

A result well under 20% is a standard clue pointing toward iron-deficiency anemia, since it shows relatively little of the metal occupying a normal or expanded carrying capacity. Ferritin, a measure of what's stored, is usually checked alongside it, since ferritin can also rise independently during inflammation and mask an underlying deficiency.

Should TSAT be interpreted on its own?

No — clinical guidelines consistently recommend reading it together with ferritin and the rest of the panel rather than in isolation. TIBC and serum levels can each shift for reasons unrelated to what the body actually has stored, including inflammation, liver disease, and certain medications, all of which can distort TSAT if considered alone.

Why is TSAT expressed as a percentage rather than a raw number?

Because it represents a proportion of a maximum — how full transferrin's binding capacity is — expressing it as a percentage makes results directly comparable across people with naturally different total iron-binding capacities. A raw ratio without the ×100 scaling step would still be mathematically valid but would not match how the figure is reported and interpreted clinically.

What units does this calculator expect?

Both serum iron and TIBC in micrograms per deciliter (µg/dL), the units standard on most panels. Since the formula is a straight ratio multiplied by 100, the units cancel and the resulting percentage is unaffected as long as both values are reported on the same scale.
