How this instrument works
The number behind this calculator is a real, published rate: twin births per 1,000 total births, broken out by the mother's age at delivery, from the CDC's National Center for Health Statistics. It comes from 'A Decade of Decline in Twin Childbearing in the United States, 2014-2024' (Martin & Osterman, NCHS Health E-Stat 117), using final 2024 natality data — the most recent year available. Six age bands are built in, from younger than 20 (16.1 per 1,000) up through 40 and older (35.6 per 1,000).
This is a population average, not a prediction about any individual pregnancy. It answers 'out of every 1,000 births to mothers in this age group nationally, how many included twins,' not 'what are the odds for me specifically.' The 'odds, expressed as 1 in N births' figure is just the same rate turned around — 1,000 divided by the rate — so a rate of 28.9 per 1,000 becomes roughly 1 in 34.6, two ways of reading identical data.
Age is deliberately the only factor modeled here, because it's the only one with a clean, citable population rate attached to it. Fertility treatment (ovulation-inducing drugs, IVF with multiple embryos transferred), a family history of fraternal twins, and maternal weight are all real, documented factors that raise the odds of a multiple pregnancy beyond the age-based baseline — but none of them come with a single, reliable multiplier that could responsibly be bolted onto the CDC's number without inventing one. Treat those as qualitative context alongside this figure, not as something this calculator already accounts for.

Worked example — mother aged 25 to 29
A mother in the 25-29 age band: CDC/NCHS Health E-Stat 117 (Martin & Osterman, final 2024 data) reports 28.9 twin births per 1,000 total births for this group. Dividing 1,000 by 28.9 gives about 34.6 — so roughly 1 in every 34.6 births to mothers in this age band includes a twin pregnancy, nationally.
Move to the 35-39 band and the reported rate rises to 35.4 per 1,000, or about 1 in 28.2 — noticeably more common. Move down to mothers younger than 20 and the rate drops to 16.1 per 1,000, or about 1 in 62.1 — the lowest of the six age-banded rates in the 2024 data. Age is a strong, well-documented factor, but it's still a population average applied to one age band, not a reading of any individual mother's biology.
Questions
Does this number predict whether I personally will have twins?
No. It's a population statistic — the share of all births to mothers in a given age band nationally that included twins, from the CDC's final 2024 natality data. It says nothing about any individual pregnancy's odds, which depend on factors like ovulation patterns, fertility treatment, and family history that aren't part of this calculation.
Why does the twin rate rise so much with age?
Two things are blended into the reported population rate: naturally occurring fraternal twinning becomes somewhat more likely with age as hormonal changes make releasing more than one egg per cycle more common, and use of fertility treatments that raise multiple-pregnancy odds is also more common among older mothers. The CDC's published rate doesn't separate these two contributions — it's a single combined population figure for each age band.
Does fertility treatment raise my odds above what this calculator shows?
It can. Ovulation-inducing fertility drugs and IVF (when more than one embryo is transferred) are established causes of multiple pregnancy — ACOG's own patient guidance names both directly. This calculator doesn't apply a fertility-treatment multiplier on top of the age-based CDC rate, because no single, reliably sourced multiplier exists for that; treat fertility treatment as a real factor to discuss with a clinician, separate from the number shown here.
Does a family history of twins matter?
It's a commonly cited factor for fraternal (non-identical) twinning specifically, since a tendency toward releasing more than one egg per cycle can run in families. It isn't reflected in this calculator's number, for the same reason fertility treatment isn't: there's no single, well-sourced multiplier for family history that could be responsibly applied to the CDC's age-based baseline.
What exactly does 'per 1,000 total births' mean?
It's a rate, not a raw count: of every 1,000 births recorded to mothers in a given age band in the reference year, this many involved a twin delivery. Expressing it this way lets age bands of very different sizes be compared on equal footing, and it's the same convention CDC/NCHS uses in its own published tables.
