How this instrument works

This tool takes a child's weight in kilograms and applies the standard pediatric acetaminophen range of 10 to 15 mg per kilogram per dose, repeated every 4 to 6 hours, with no more than five doses in a 24-hour stretch. It is an educational reference for working the arithmetic, not a substitute for medical judgment. Confirm any dose with your child's pediatrician or a pharmacist before giving acetaminophen, measure only with the dosing syringe or cup that came in the product box — never a kitchen spoon, which can vary by a factor of two or more — and if you suspect an overdose or are unsure about a dose already given, call Poison Control at 1-800-222-1222 (US) or emergency services right away.

The 160 mg per 5 mL figure this calculator assumes is the concentration now printed on nearly every US brand of children's liquid acetaminophen. That wasn't always true. Before 2011, stores stocked two different strengths side by side: a concentrated 'infant drops' formula near 80 mg per 0.8 mL, and a more dilute 160 mg per 5 mL 'children's liquid' meant for older kids. Caregivers occasionally reached for the wrong dosing chart for the bottle in hand, and that mismatch caused real, sometimes fatal, overdoses. Manufacturers, working with the Consumer Healthcare Products Association and the FDA, voluntarily retired the stronger infant drops and moved every product onto the single 160 mg/5 mL strength this page assumes. Before dosing by volume, check the concentration printed on the actual bottle in hand — older stock or an unusual product could still differ from that now-standard number.

Two ceilings sit on top of the per-dose range. The first is 75 mg per kilogram per day; the second is a flat 4000 mg per day regardless of size. A small child's daily limit is set by the weight-based figure, but past roughly 53 kg the flat 4000 mg line becomes the binding one — this calculator always applies whichever cap is lower, and a caregiver reading only the per-kilogram arithmetic could otherwise stack doses past the safer flat limit.

Worked example — 15 kg child

A 15 kg child: low end is 10 × 15 = 150 mg, high end is 15 × 15 = 225 mg per dose. Converted to syrup at 160 mg/5 mL, that's 150 ÷ 160 × 5 = 4.69 mL on the low end and 225 ÷ 160 × 5 = 7.03 mL on the high end — the range a dosing syringe would actually be filled to.

The 24-hour ceiling for this child is the lesser of 75 × 15 = 1125 mg and the flat 4000 mg line, so 1125 mg governs — roughly five doses at the high end before the daily limit is reached. Scale up to a 60 kg child and the weight-based figure (75 × 60 = 4500 mg) would exceed 4000, so the flat cap takes over instead: 4000 mg becomes the binding 24-hour limit, sitting under a per-dose range of 600-900 mg (18.75-28.13 mL).

What happened to the old 'infant drops' concentration of Tylenol?

It was voluntarily phased out in 2011. Before then, a separate 'infant drops' formula near 80 mg per 0.8 mL sat on shelves next to the more dilute 160 mg per 5 mL children's liquid, and using one product's dosing chart on the other's bottle caused real overdoses — some fatal. Why does this calculator show a range instead of one number?

Because the clinical guidance itself is a range — 10 to 15 mg/kg per dose — and a prescriber or caregiver picks a point inside it depending on how the fever or pain is responding. Ten times the child's weight is a reasonable starting figure for routine symptoms; the higher end, 15 times weight, is used for more severe cases, always still bounded by the same 24-hour ceiling.

Why does the 24-hour maximum stop rising with weight at some point?

Because two ceilings apply, and the calculator takes whichever is lower. The weight-based cap, 75 mg/kg/day, keeps climbing with size, but a flat 4000 mg/day limit exists independent of weight. Past about 53 kg, that flat number becomes lower than the weight-based one and takes over — a heavier child or small adult does not get an unlimited daily total just because the per-kilogram math would allow more.

Can I measure a dose with a kitchen spoon if I don't have the syringe?

No — never a household spoon. Kitchen teaspoons vary enormously in actual volume, and caregiver dosing-error studies keep finding spoon-measured doses running well outside the intended range in either direction. Use only the oral syringe or dosing cup packaged with the product; if it's been misplaced, a pharmacist will hand you a replacement rather than have you guess with flatware.

How often can acetaminophen doses be repeated?

Every 4 to 6 hours, with no more than five doses in a 24-hour period. Spacing doses closer than that — even by a little, when a fever is stubborn and a caregiver is tempted to give 'just a bit early' — is one of the more common ways a child ends up pushed past the daily ceiling this calculator works out.
