How this instrument works
The AST-to-Platelet Ratio Index divides a patient's AST level by their lab's own upper limit of normal for AST, then divides that ratio by the platelet count and multiplies by 100. Wai CT, Greenson JK, Fontana RJ and colleagues built it as a noninvasive alternative to liver biopsy, publishing their validation in Hepatology in 2003, using data from patients with chronic hepatitis C.
In that original study population, an APRI at or above 1.0, and especially at or above 1.5, was associated with meaningfully higher odds of significant liver fibrosis or cirrhosis. The appeal is that both inputs, AST and platelet count, already sit on a routine blood panel, so no additional test or procedure is needed to get a first read on fibrosis risk.
APRI was developed and validated specifically in chronic hepatitis C patients, and its accuracy in other liver-disease contexts is less established; it does not carry the same evidence base for, say, fatty liver disease or alcohol-related liver injury. A raised APRI is a reason for further clinical evaluation, not a diagnosis on its own — liver fibrosis staging involves imaging, other blood markers, and sometimes biopsy alongside a score like this one.

Worked example — three AST-and-platelet pairs
An AST of 80 U/L against a lab's upper limit of normal of 40 U/L gives a ratio of 80 ÷ 40 = 2. Divide by a platelet count of 150 (×10⁹/L): 2 ÷ 150 = 0.01333. Multiply by 100 and APRI comes out to about 1.33 — above the 1.0 threshold, and close to the 1.5 threshold, the original study associated with meaningfully raised odds of significant fibrosis or cirrhosis.
A normal AST sitting exactly at its own upper limit of normal, 40 against 40, gives a ratio of 1. Divide by a normal platelet count of 250: 1 ÷ 250 = 0.004, times 100, for an APRI of exactly 0.4, comfortably under the threshold the original study flagged as low-risk.
An AST of 120 U/L, three times its 40 U/L upper limit of normal, gives a ratio of 3. Divide by a low platelet count of 80, itself a marker sometimes associated with portal hypertension: 3 ÷ 80 = 0.0375, times 100, for an APRI of 3.75, well into the range the original study associated with a high probability of cirrhosis.
Questions
What does an APRI score above 1.0 mean?
In the population the index was validated in, chronic hepatitis C patients, an APRI at or above 1.0 was associated with meaningfully higher odds of significant liver fibrosis, and at or above 1.5, higher odds of cirrhosis specifically. It is a risk-stratification signal from the original 2003 study, not a diagnosis, and it should prompt further evaluation rather than stand alone as an answer.
Can APRI replace a liver biopsy?
No — it was developed as a noninvasive alternative that could reduce how often biopsy is needed, not eliminate it. Wai and colleagues built APRI specifically so that two numbers already on a routine blood panel, AST and platelet count, could give a first estimate of fibrosis risk in chronic hepatitis C, but biopsy or other confirmatory workup still has a role when the picture is unclear.
Does APRI work the same way for liver diseases other than hepatitis C?
Its accuracy is far less established outside chronic hepatitis C, the population it was developed and validated in back in 2003. Applying an APRI score to fatty liver disease, alcohol-related liver injury, or other causes should be done cautiously, if at all, since the fibrosis-to-score relationship was built on a different patient group.
Why does the lab's own upper limit of normal for AST matter?
Because different laboratories calibrate their normal ranges differently, so the same raw AST number can represent a different degree of elevation depending on where it was measured. Dividing AST by that lab's own upper limit of normal, rather than by a fixed figure, keeps the ratio meaningful across labs with different reference ranges.
Why does a low platelet count raise the APRI score?
Because platelet count sits in the denominator; a lower count divides the AST ratio less, so a low platelet count alone can push APRI higher. Low platelets are also, independently, a recognized sign of possible portal hypertension in liver disease, part of why the original study found raised AST paired with low platelets meaningful together.
