How this instrument works
Cost per mile is the simplest possible rate: whatever you spent, divided by however far you drove to spend it. Worked example — $45 of expense over 300 miles
A vehicle owner tracks $45 in total expense (fuel plus a routine top-up) against 300 miles driven over the same stretch. Entering 45 for total cost and 300 for total miles gives cost per mile = 45 / 300 = $0.15 per mile.
That $0.15/mi rate can now be used to estimate other trips in the same vehicle under similar conditions — a 120-mile weekend trip would project to roughly 120 x $0.15 = $18.00, without needing to re-track fuel receipts for every new distance.

That depends entirely on what you're trying to measure, and this instrument works the same way either way. For a quick fuel-only rate, total cost is just what you spent on gas. For a fuller ownership-cost rate, total cost could also include maintenance, insurance, registration and depreciation over the same period — closer to what the IRS standard mileage rate is trying to approximate. Decide what you're measuring first, then make sure the miles you enter match that same scope and time period.

Does a lower cost per mile always mean a more efficient vehicle?
Not necessarily on its own — cost per mile reflects whatever you entered as total cost, which might be fuel-only, might include maintenance, or might reflect a short trip with unusually cheap or expensive gas. A fair efficiency comparison between two vehicles needs the same scope of cost (e.g. fuel-only vs. fuel-only) measured over a similar type of driving, since a highway-heavy trip and a stop-and-go city trip in the same car can produce noticeably different cost-per-mile figures.
