How this instrument works
Quart is the older word by several centuries — English law was regulating quarts of wine and ale while a cup was still just something you drank from. What the United States inherited is a quarter of the wine gallon of 231 cubic inches, which puts one US liquid quart at 57.75 cubic inches, or 946.352946 mL exactly once the 1959 yard and pound agreement pinned an inch at 25.4 mm. Its cup arrived far later: an American kitchen habit that hardened into a legal unit of 8 US fluid ounces, two gills, a quarter of that quart.
So the factor is purely definitional. A quart holds 32 US fluid ounces and a cup holds 8, meaning four fill one by construction — nobody ever poured a vessel and compared. Multiplying by four is also about the gentlest thing you can ask of a computer, since 4 is a power of two: your machine bumps an exponent and leaves every significant bit alone. Type 0.1 into that quart field and 0.4 comes back carrying exactly the rounding error it arrived with, plus nothing.
Between them these two divide an American kitchen. Quarts describe capacity and packaging — buttermilk, stock cartons, engine oil bottles, ice cream churns, and cookware above all, from a 3-quart saucier through a 6-quart Dutch oven to an 8-quart stockpot. Cups describe portions, because portions are what recipes count in. Both live on one ladder of halvings, gill to cup to pint to quart to gallon, which keeps American kitchen arithmetic mercifully free of long division.

Worked example — filling a 3-quart pot
Your saucepan is stamped 3 quarts and your chowder recipe counts in cups. Enter 3 into Quarts (qt) and US cups (cup) reads 12.0 — which is also 96 US fluid ounces, 173.25 cubic inches, and 2.839058838 litres of soup. Note what that answer implies: a 12-cup recipe fills such a pan to its brim, leaving no freeboard for a simmer, so reach for the next size up.
Nothing rounds anywhere along that path. One quart is 4 × 236.5882365 mL, three of them come to 2839.058838 mL, and twelve customary cups reproduce that figure digit for digit. Scale down by a third and 1 qt gives 4 cups; double instead and 6 qt gives 24, at which point your stockpot decides the recipe.

Is a quart of berries the same as a quart of milk?
No. Produce is sold by dry measure, and a US dry quart is 67.200625 cubic inches — 1101.22 mL, about 16% above its liquid cousin — because dry units descend from the Winchester bushel rather than from the wine gallon. My nutrition label says a cup is 240 mL — does that change the count?
Slightly, and it is worth knowing which one you hold. Federal food labelling rounds a cup to 240 mL so serving sizes print cleanly, while measuring sets and cookbooks use 236.5882365 mL. Divide a quart by that label cup and you get 3.94 rather than 4 — a 1.4% gap, about three teaspoons per quart. Invisible in stew, plainly visible across a canning batch or a bottled syrup. What size pot does a 12-cup recipe need?
Larger than 3 quarts. Twelve cups is exactly 3 qt, and cookware is rated by level brim capacity, so a batch equal to its rating will boil over the moment it moves. Allow a third again for anything that foams or reduces: 4 quarts for 12 cups of stock, more for beans or pasta.
