How this instrument works
The testosterone-to-estradiol ratio, T/E2 for short, is one figure some clinicians track in men on testosterone replacement therapy. It is a rough proxy for aromatization — the enzyme aromatase converts a slice of circulating testosterone into estradiol, and on TRT the raw material for that conversion rises sharply along with the dose. A figure that drops too low, the reasoning goes, flags a man converting an unusually large share of his testosterone into estrogen, which can show up as fluid retention, tender breast tissue, mood swings, or blunted libido. The same division occasionally surfaces on the other side of the clinic, in hyperandrogenism or PCOS workups, where an elevated T/E2 is one data point among several rather than a diagnosis by itself.
This instrument divides total testosterone in ng/dL by estradiol in pg/mL without converting either one to match the other first — and that is deliberate, not sloppy arithmetic. Convert testosterone into estradiol's own unit properly and the two figures sit on wildly different footing: 500 ng/dL of testosterone works out to roughly 5000 pg/mL, two orders of magnitude above a typical estradiol reading, which would turn the output into a number nobody in reproductive medicine actually reports. Clinics instead take each lab value exactly as the assay returns it and divide, producing an index that tracks change within one person over time rather than a physically meaningful proportion between two matched quantities. Treat the result as a monitoring figure, not a unit of anything.
No major guideline fixes a single target for this number. The Endocrine Society's 2018 clinical practice guideline on testosterone therapy recommends checking estradiol selectively — when gynecomastia, mood changes, or sexual side effects appear — rather than as routine follow-up alongside testosterone and hematocrit, and it stops short of endorsing any cutoff. A 2021 study of men treated for hypogonadotropic hypogonadism found that a value near 12 separated patients with adequate erectile response from those without, but that came from one small cohort on one treatment protocol, measured on one lab's assay, and has not been validated as a general target. Figures that circulate informally online — a healthy band of roughly 10 to 30 shows up often — are descriptive averages, not proven clinical thresholds.

Worked example — 500 ng/dL testosterone, 25 pg/mL estradiol
Total testosterone of 500 ng/dL over an estradiol of 25 pg/mL: 500 ÷ 25 = 20. No unit conversion happens first — the 500 stays in ng/dL, the 25 stays in pg/mL, and the division runs on the two figures exactly as the report shows them. A result of 20 sits inside the band some clinics treat as reassuring against excess aromatization, though no controlled study has proven that exact boundary meaningful.
Drop testosterone to 300 ng/dL and raise estradiol to 40 pg/mL, and the output falls to 300 ÷ 40 = 7.5 — a lower testosterone reading and a higher estradiol reading both pull the number down, exactly the pattern a clinician managing aromatization watches for. Push testosterone up to 700 ng/dL against a lower estradiol of 20 pg/mL instead, and the figure climbs to 700 ÷ 20 = 35, the direction a well-controlled dose typically produces.
Questions
What does a T/E2 ratio actually tell a clinician?
It is a rough proxy for how much of a man's testosterone is aromatizing into estradiol relative to how much testosterone is circulating. A low value suggests heavy conversion to estrogen, which tracks with side effects like gynecomastia or fluid retention in some men on TRT; a higher value suggests comparatively less aromatization. It is one monitoring figure among several — symptoms and absolute hormone levels still matter — not a stand-alone diagnosis.
Why doesn't the calculator convert testosterone and estradiol to matching units first?
Because that is not how clinics report the figure. Testosterone in ng/dL divided by estradiol in pg/mL, mismatched units and all, is the conventional T/E2 calculation used in TRT monitoring. Converting first would produce a completely different, far larger number that nobody in practice actually tracks — the mismatched-unit version is the one that appears on lab summaries and in the clinical literature.
Is there an ideal T/E2 value to aim for?
Not one that any major guideline has endorsed. Figures like 10 to 30 circulate as rough, informal descriptions of a healthy band, and one 2021 study of men treated for hypogonadotropic hypogonadism found a cutoff near 12 separating better and worse responders — but that came from a single small cohort, not a validated clinical target. The Endocrine Society's own testosterone therapy guideline does not specify a ratio at all.
Does this same calculation apply outside of testosterone therapy?
A similar division shows up occasionally in women's hormone workups — hyperandrogenism or PCOS evaluations sometimes reference a testosterone-to-estradiol figure — but it is far less standardized there than in TRT monitoring. This instrument is built around the TRT use case, where total testosterone in ng/dL and estradiol in pg/mL are the values a routine follow-up panel already produces.
Why might this number shift between visits on an unchanged dose?
Assay variability is the usual culprit. Different labs run different estradiol assays with different sensitivity, particularly at the low concentrations typical in men, and a single draw can swing meaningfully between visits for reasons unrelated to the actual dose. Trend the figure across several visits at the same lab rather than reacting to any one reading.
Should a dose change be based on this figure alone?
No — treat it as one input for a conversation with the prescribing clinician, not a self-adjustment trigger. Dose changes, aromatase-inhibitor decisions, and injection frequency all interact with this number in ways a single division cannot capture, and reported symptoms carry at least as much weight as the arithmetic itself.
