How this instrument works

Home urine pregnancy tests detect human chorionic gonadotropin (hCG), a hormone the placenta starts producing shortly after implantation. Every test has an analytical sensitivity — the lowest hCG concentration, in mIU/mL, it can reliably detect. A lower number means a more sensitive test, able to pick up a positive result earlier in a pregnancy, when hCG is still low. This calculator takes an hCG level and checks it against one of two measured sensitivity thresholds, then reports the margin: how far above or below that threshold the level falls.

The two thresholds here aren't manufacturer marketing claims — they're measured values from Cole LA, Khanlian SA, et al., published in Clinical Chemistry and Laboratory Medicine (2011), which directly tested six over-the-counter test lines against known hCG concentrations. First Response manual and digital tests measured an analytical sensitivity of 5.5 mIU/mL, correctly detecting 97% of 120 pregnancies on the day of missed menses. EPT and Clearblue manual and digital tests measured a substantially higher — meaning less sensitive — threshold of 22 mIU/mL, and in the same study detected only 54-67% of those same pregnancies on that day, well below the greater-than-99%-type detection claims those brands have advertised.

That gap matters most in the days right around a missed period, when hCG is still rising and hasn't yet cleared every test's threshold. A level of 15 mIU/mL, for instance, is comfortably above zero and reflects a real, ongoing pregnancy, but it would still read negative on a 22 mIU/mL-threshold test while reading positive on a 5.5 mIU/mL-threshold test — not because anything is wrong, but because the two tests are built to different sensitivities.

  • Enter the hCG level in mIU/mL — from a blood test result, or a level being used hypothetically to compare test sensitivities.
  • Select the test brand-tier: a high-sensitivity test (about 5.5 mIU/mL, e.g. First Response) or a standard-sensitivity test (about 22 mIU/mL, e.g. EPT or Clearblue).
  • Read the margin — how many mIU/mL the entered level sits above or below that test's measured threshold.
  • Read the likely-result flag, which shows whether the level meets or exceeds that specific threshold.

Worked example — 50 mIU/mL against a high-sensitivity test

An hCG level of 50 mIU/mL checked against a high-sensitivity, First Response-tier test (5.5 mIU/mL threshold) clears that threshold by 44.5 mIU/mL: 50 minus 5.5. The likely-result flag reads positive — 50 mIU/mL is well above the level that study found this test-tier reliably detects.

Drop the level to 15 mIU/mL and check it against a standard-sensitivity, EPT/Clearblue-tier test (22 mIU/mL threshold) instead: the margin comes out negative, 15 minus 22 equals -7 mIU/mL, and the flag reads likely negative. That's a real, nonzero hCG level that would still plausibly not register on this less-sensitive test-tier at this concentration.

Raise that same test-tier's input slightly, to 30 mIU/mL against the 22 mIU/mL threshold, and the margin flips positive: 30 minus 22 equals 8 mIU/mL, likely positive. A few mIU/mL of difference in the underlying hormone level is exactly the range where a standard-sensitivity test can go from missing a pregnancy to catching it.

Questions

Is this calculator a substitute for taking an actual pregnancy test?

No. This is an informational tool, not a diagnostic one. It compares a number you already have — an hCG level, typically from a blood test — against published sensitivity data for two test-tiers; it does not perform a test and cannot tell you your actual hCG level. Any home test result, positive or negative, should be confirmed with a healthcare provider, especially if it doesn't match what you expected.

Why do EPT and Clearblue tests need a higher hCG level than First Response to register positive?

Because Cole et al. (2011) measured their analytical sensitivity — the lowest concentration they reliably detect — at 22 mIU/mL, versus 5.5 mIU/mL for First Response manual and digital tests. That's a real, measured difference in how the test strips are manufactured, not a reflection of anything about a given pregnancy. A test with a higher threshold simply needs more hormone present before it reliably shows a positive line.

Does this mean EPT and Clearblue tests don't work?

No — they work, just at a different sensitivity than some competitors and, per this same study, somewhat below their own advertised claims: 54-67% detection versus a marketed greater-than-99% figure, specifically on the day of missed menses, when hCG is often still climbing. Waiting a few more days after a missed period, when hCG is typically much higher, meaningfully increases the odds any test in this study would catch a true pregnancy.

How current are these sensitivity numbers?

They come from one study, conducted at one point in time (published 2011), testing specific product formulations available then. Manufacturers can and do change test formulations, and this calculator doesn't have a way to verify whether a given box on a store shelf today matches exactly what was tested. Treat the numbers here as a documented reference point, not a live specification for whatever test is currently in a given package.

If my hCG level is below both thresholds, does that mean I'm not pregnant?

Not necessarily. Early pregnancy hCG rises quickly but starts near zero, so a low level can simply mean testing happened very early. It could also reflect a different clinical picture entirely. A single number below a threshold is not a diagnosis — a healthcare provider, working from an actual test result and the full clinical picture, is who can interpret what a given level or a negative test result actually means.

Why isn't there a third, lower-threshold option for other test brands?

Because this calculator only uses figures that trace directly back to a primary, peer-reviewed source — the Cole et al. 2011 abstract. A commonly cited 'ultra-sensitive, about 100 mIU/mL' figure for some other brands turns up in secondary, non-peer-reviewed sources, but it couldn't be confirmed against a primary study, so it was deliberately left out rather than presented as if it carried the same evidence weight as the other two numbers.
